A counselor can speak with your patient over the phone about the risks presented by drugs, medications, infectious diseases or other agents during pregnancy.

How can I refer my patients to the CTIS Pregnancy Risk Information Line?

1. Have your patient call (800) 532-3749. To order brochures for your patients, click here

2. Have your patient email a counselor at ctispregnancy@ucsd.edu or
use the email a counselor form

3. Have us call your patient through our fax referral form service. (Service provided for California residents only).

Would you like us to call your patient?

Here’s how:

1. Print the FAX Referral Form, fill-in the required information, have your patient sign the consent to be contacted.

A written summary letter of the current known information about her exposures can be sent to her.

Your patient can provide a copy of this letter to you at her first or subsequent visit, which can be included in her medical file, or she can sign the consent form to have a copy of her letter sent to you.

 2. Fax the form to (619) 220-0228.

Many California Obstetricians, Nurse-Midwives, Public Health Nurses, and most San Diego WIC sites and Planned Parenthood Prenatal Clinics use this procedure to help educate their patients/clients.
