Would you like us to call your patient?
FAX a Referral Form to a Teratogen Information Specialist (service provided for California residents only):
A Teratogen Information (TI) Specialist can contact your patient for free, confidentially, over the phone about the pregnancy risks presented by drugs, medications, infectious diseases or other agents.
Here’s how:
- Print the FAX Referral Form, fill-in the required information, have your patient sign the consent to be contacted.
- A written summary letter of the current known information about her exposures can be sent to her.
- Your patient can provide a copy of this letter to you at her first or subsequent visit, which can be included in her medical file, or she can sign the consent form to have a copy of her letter sent to you.
- Fax the form to (619) 220-0228. California Obstetricians, Nurse-Midwives, Public Health Nurses (PHNs), most San Diego WIC sites, and Planned Parenthood Prenatal Clinics in San Diego use this procedure to get the education to their pregnant patients/clients.
CTIS Pregnancy Follow-up Program:
Pregnant women who call the service are asked if they would be willing to participate in the ongoing CTIS studies throughout their pregnancy and post-partum. Participation in CTIS studies increases much needed data on drug exposures in pregnancy. Participation in the studies include:
- Free specialized pediatric examinations that are available for infants/babies of women participating in CTIS’ follow-up program.
- Free developmental testing that may be offered for the child.
- For more information, go to our RESEARCH page.
- If you would like to request brochures please click HERE and submit your information on-line (Litmit of 200).
