FAX a Referral Form to a Teratogen Information
Specialist
Let CTIS Pregnancy Risk Information Line do the work! A Teratogen Information
(TI) Specialist can contact your (California ONLY) patient for free, in
strict confidence, over the phone about the pregnancy risks presented
by drugs, medications, infectious diseases or other agents. Simply print
the FAX Referral Form (download
PDF file), fill-in the required information, have your patient consent
to be contacted and then, fax the form to the CTIS main office. This procedure
is currently being used by California Obstetricians, Nurse-Midwives, Public
Health Nurses (PHNs), most San Diego WIC sites, and Planned Parenthood
Prenatal Clinics in San Diego and is effective in getting the education
to pregnant women. Additionally, a written summary letter of the current
known information about her exposures can be sent to her. In turn, your
patient can provide a copy of this letter to you at her first or subsequent
visit which can be included in her medical file, or she can sign the consent
form to have a copy of her letter sent to you.
Refer your patient to CTIS
Simply instruct your patient to contact CTIS immediately after they have
set their first appointment with your office. The toll-free number is
1-800-532-3749. Having them call allows CTIS to do an “exposure
check-up” and provide them educational information before they see
their provider. Thus, by the time of their visit to your office your patient
has had the opportunity to be educated about her exposure, and her fears
or misconceptions can be addressed. A written summary letter of the current
known information about her exposures can be sent to her. In turn, your
patient can provide a copy of this letter to you at her first or subsequent
visit which can be included in her medical file.

CTIS Pregnancy Follow-up Program
CTIS also has a Pregnancy Follow-up Program. Pregnant women are encouraged
to participate in this confidential program throughout their pregnancies,
and post-partum. This allows CTIS to collect valuable data that adds to
the current body of scientific information concerning potential teratogens.
Free specialized pediatric examinations are available for infants/babies
of women participating in CTIS’ follow-up program. Also, free developmental
testing may be offered for the children when he/she reaches school age.
Teratogen Information Specialists (bilingual in English, Spanish or Tagalog)
are available weekdays from 9:00 A.M. to 5:00 P.M. to answer questions
from
pregnant women, women preparing for pregnancy, and health care providers.
